PATIENTS PERSPECTIVE

To achieve the goals identified from the patient’s perspective, 4-HNS suggests three simple steps:

The algorithm will guide the patient to schedule a video conference. When proceeding in this way, the patient actively seeks contact with the attending physician. The virtual meeting facilitates the first contact and offers a preliminary interface to promote empathy and belonging. At the same time, the attending physician begins to develop a care plan, always sharing evidence-based medicine and integrative care with the patient. Little by little, the patient will understand what comprehensive care means, assuming a cooperative attitude, being proactive and participating in the introduction of a patient-centric care model.

STEP 1

Promote proactive patient behavior from the first contact

This step will take place in a face-to-face consultation, which is essential in clinical care. The personal approach reinforces doctor-patient relationship and allows the physical examination to be carried out. The doctor will then record patient’s clinical data in their personalized Digital Health Record (DHR). The algorithm directs the attending physician to approach integrative medical care supported by a digitized clinical problem-solving methodology.

STEP 2

Use DHR-embedded clinical problem-solving

The attending physician appoints all consulting physicians necessary for patient’s comprehensive care, creating a Customized Care Team (CCT). Each CCT is very specific and should work with collective intelligence. The attending physician becomes the conductor and ensures its fine-tuning. Working together, CCT will create and complete a Customized Care Program (CCP). Click on the acronyms below to see templates suggested by the 4-HSN platform.

An agenda is created, where the patient has access to a calendar of appointments, exams, and incidental findings. The attending physician will fill it out, indicating the programmed date (P-date) for each intervention and the actual completion date (C-date). Remarks, when checked with (Yes), will be described in full in the patient’s DHR, pointing out the need for additional investigation. The inclusive nature of 4-HSN will give the patient visibility into the updated completion of their CCP in real time and anywhere in the world. The table illustrates the patient’s interface.

Each clinical case will require an attending physician and consulting physicians. The number of consulting physicians varies depending on the complexity of each clinical case. The inclusive nature of 4-HSN will give the patient visibility into the composition of the assigned CCT in real time and anywhere in the world. The table illustrates the patient’s interface.

We collect name, e-mail, telephone number, academic background, educational institution, year of completion, work experiences, and other information contained in your resume, whenever you spontaneously disclose them as attending physician, consultant physician, or registered nurse. We collect name and password when you log in to access any restricted area created by 4-HSN participation. We also collect the data you provide when you interact with us via email or WhatsApp. We do not recommend that you provide personal data in the body of the message, but if you do, it will also be collected. Remember that you are also responsible for the care of your personal data and the personal data of those you share.

 

WHY DO WE COLLECT YOUR DATA AND HOW LONG DO WE KEEP IT?

We collect your personal data to respond to your question or comment posted in the contact tab. When communicating with physicians, their data is collected for propagation and referral. Personal data processed is used for the time necessary to fulfill the purposes described in this policy. When the interaction is no longer required, data are securely deleted.

Your personal data will be made available to third parties, suppliers and partners, when necessary for some of the activities mentioned in this policy.
